Once the patient has returned to the home setting, our agency assists patients and caregivers in the transition from hospital to home. Our staff helps patients adjust and overcome challenges they will face regarding daily living with a new ostomy and/or care of their wounds.
We are a fully licensed and certified home health agency. We provide in-home wound and ostomy care, assessment, and teaching for home-bound patients who experience a taxing effort when leaving their homes. Upon receipt of a physician’s order, our staff will come to the home to evaluate the patient’s needs and make recommendations to the physician regarding the best way to meet the specific needs of the patient and caregivers.
